Community Care Specialist

At SE Health, we're seeking a dedicated and compassionate Community Care Specialist to join our team. This is a permanent full-time position that offers a guaranteed 37.5 hours per week with 12-hour shifts.
About the RoleIn this role, you'll provide comprehensive care in an interdisciplinary team setting. You'll be the primary contact for clients when designated as the primary care provider/clinician and provide continuous support to your team in delivering care. Your responsibilities will include completing client assessments, developing interdisciplinary care plans, leading care planning processes, and facilitating regular interdisciplinary team communication.

About YouTo succeed in this role, you'll need a Bachelor's degree in nursing (or equivalent) and minimum two years of nursing experience. Canadian Nurses Association (CNA) community nursing certification is an asset. You should have diverse nursing experiences with different client groups and excellent skills in case management and working with/coordinating interdisciplinary care teams. A vehicle and valid driver's license are also required.
