Job Summary

Halton Healthcare is seeking a skilled clinician to join our Adult Mental Health Day Program as a Clinical Case Manager. The successful candidate will possess solid knowledge and experience in working with individuals with mental illness, with a focus on crisis management, symptom stabilization, education, group and individual counseling, and coordination of resources for follow-up after discharge.

Key Responsibilities
  • Provide assessments and treatment planning for clients in collaboration with an interdisciplinary team.
  • Develop and implement groups, demonstrating strong group facilitation skills.
  • Liaise with community partners, family physicians, and case managers to advocate for this population.
  • Develop programming based on best practices.
  • Utilize strong interpersonal and leadership skills to work collaboratively within the team.
  • Adapt to the demands of a constantly changing workload.
Requirements
  • Current college registration (RN, MSW, OT) is required.
  • Strong problem-solving and conflict/crisis resolution skills are essential.
  • A working knowledge of the Mental Health Act and other related legislation is necessary.
  • Excellent communication and interpersonal skills are required.
