Treatment Navigator Job Summary

The Treatment Navigator plays a vital role in supporting clients in their recovery journey, empowering them to maximize their social and economic potential and achieve their goals within our addiction treatment programs.

Key Responsibilities:
  • Participate in team efforts to maintain and improve the intake, admissions, and orientation process.
  • Maintain client confidentiality and respond to client rights to healthcare information.
  • Assess clients' suitability for the program and make referrals to other agencies when necessary.
  • Work with the integrated addiction services team on clients' case planning and concerns.
  • Maintain accurate client records, including assessment data, client care plans, goal setting, case notes, and discharge summaries.
  • Act as a positive role model, coach, advisor, leader, and rational authority for clients.
  • Take new clients and collaborate with other staff members to identify acute and chronic mental health problems faced by clients.
  • Develop treatment plans and strategies to best meet client needs, individually and in consultation with primary treatment providers.
  • Assist clients with completing instrumental daily activities, including household upkeep and community participation.
  • Observe client behavior and maintain appropriate documentation, reporting changes in physical or mental health to the clinical team.
Qualifications:
  • Grade 12 plus graduation from a recognized Mental Health Care Worker Program or an equivalent combination of education, training, and experience.
  • 1 year of experience working with vulnerable populations.
  • Valid Class 5 driver's license with a clean driver's abstract.
  • First Aid Certificate with CPR: current.
  • Valid Foodsafe certificate.
  • Satisfactory completion of Employee Immunization Record form is required.
Skills and Knowledge:
  • Ability to communicate effectively both verbally and in writing.
  • Ability to deal effectively with others.
  • Ability to teach.
  • Physical ability to carry out the duties of the position.
  • Ability to organize work.
  • Ability to operate related equipment.

The salary for this role is between $27.92 and $29.60 per hour.
