Treatment Navigator

We are seeking a highly skilled and compassionate Treatment Navigator to join our team at Phoenix Society. As a key member of our addiction treatment team, you will play a vital role in guiding clients towards recovery and enhancing their potential for success.
Key Responsibilities- Client Intake and Assessment: Participate in team efforts to streamline intake, admissions, and orientation processes, ensuring a smooth transition for clients into our treatment programs.
- Client Support and Guidance: Assess clients' suitability for the program, make referrals when necessary, and collaborate with the addiction services team on case planning and management.
- Client Records and Reporting: Maintain accurate client records and reports, documenting client behavior and reporting any health changes to the clinical team.
- Mentorship and Leadership: Act as a mentor and leader for clients, assisting in their daily activities and mental health support, and developing treatment plans tailored to individual client needs.
- Education and Experience: High School diploma and completion of a Mental Health Care Worker Program, with a minimum of 1 year of experience working with vulnerable populations.
- Skills and Knowledge: Effective verbal and written communication skills, strong interpersonal and organizational abilities, and a valid Class 5 driver's license and First Aid Certificate with CPR.
