Job Summary

The Phoenix Society is seeking a highly skilled and compassionate Program Navigator to join our team. As a Program Navigator, you will play a vital role in supporting individuals with mental health and substance use challenges to achieve their goals and live fulfilling lives.

Key Responsibilities:

  • Advocate for residents of the Phoenix Intensive Residential Treatment Program (IRT)
  • Provide discharge planning services, including housing search and resource referrals
  • Ensure transportation for IRT clients and assist with acquiring personal documentation
  • Support individuals through crisis situations, including tenancy breakdown
  • Develop and implement individualized plans to support clients in achieving their goals
  • Collaborate with community partners and healthcare professionals to ensure comprehensive care
  • Provide education and consultation to clients, families, and healthcare professionals
  • Make effective referrals and develop, coordinate, and deliver psycho-educational and support groups

Requirements:

  • Post-secondary diploma in social sciences, counselling, social work, or a related field
  • Minimum one year of experience working with marginalized populations, substance use services, mental health, corrections, or a related field
  • Ability to administer Narcan and respond effectively to overdose situations
  • Satisfactory completion of a criminal records check with vulnerable sector screening
  • First aid certification with CPR
  • Valid class 5 driver's license
