Community Health Care Manager

About the Role:
We are seeking a seasoned Registered Nurse to lead our Neighborhood Health Team as the Community Health Care Manager - Aging in Place Specialist. This role offers an exciting opportunity to collaborate with a regional team, oversee in-home care services, manage client programs, and drive business growth.
Responsibilities:
- Direct and monitor the delivery of in-home services and daily functions of field staff, providing clinical advice, support, and resources.
- Lead sales process, business development, client acquisition strategy, and marketing activities for your NHT.
- Buildeviatestrategicandcollaborative relationshipswithcurrent,new,andprospectivepartnersandcommunityorganizations.
- Assess and document community needs while developing business and funding proposals to meet emerging opportunities.
- Develop comprehensive plans for clients and families using a holistic approach to help them achieve their aging in place goals.
- Hire and train all NHT staff members, including training and orientation on all services, products, and pricing; supervise day-to-day performance and provide ongoing coaching and performance management.
Candidates Must Have:
- A current and valid Registered Nurse (RN) registration with BCCNM
- Minimum of five years of recent experience, preferably in a community setting, with at least two years managing others
- Current and valid CPR certification
- Experience in community work and working with community partners in a collaborative way
- Experience with business development and engaging with municipal and provincial groups
- A reliable vehicle and willingness to travel to visit clients within the community (60% in the field)
- Solid knowledge of program development, implementation, and evaluation principles
What We Offer:
- Estimated salary range: $95,000 - $110,000 per annum
