About the Role

Vancouver Coastal Health is seeking a dedicated Registered Nurse to join our Home Health team in North Vancouver. As a Community Health Nurse, you will work in partnership with clients and families to establish goals, assess, plan, coordinate, implement, and evaluate nursing and other services.

Key Responsibilities

  • Assess service eligibility and develop care plans that are safe, realistic, and reasonable.
  • Deliver direct care and provide care management to assist clients in managing their own care and navigating community services.
  • Work in consultation with other system partners and incorporate current evidence into practice.

About Home Health Programs

Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative, or rehabilitative health care needs.

Client Population

Home Health clients typically have ongoing health issues and/or complex personal and/or social issues requiring care planning and coordination of appropriate resources.

Qualifications

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) is required, along with one year recent related clinical experience in a relevant clinical area.

Knowledge and Abilities

  • Demonstrated knowledge and skills in community health nursing theory and practice within a client and family-centered model of care.
  • Assessment and intervention skills, including trauma-informed, harm reduction, and culturally safe care.
  • Critical thinking and problem-solving abilities, including management of psychosocial and behavioral issues.
