As a Clinical Lead for our Care Team, you will play a key role in supporting our team of Personal Support Workers (PSWs) in providing exceptional care to our clients in their homes and communities.
Key Responsibilities:
- Conduct home visits and develop client care plans based on individual needs and authorized services.
- Teach PSWs evidence-based practices, including special functions and delegation.
- Monitor client safety and quality of care.
- Provide in-service training and education to PSWs.
- Collaborate with interprofessional teams, including Home and Community Care Support Services and other healthcare professionals.
Requirements:
- Registered Practical Nurse (RPN) or Registered Nurse (RN) with CNO registration.
- Member in good standing of the College of Nurses.
- Minimum 2-3 years of experience in community health.
- Demonstrated ability to assess client needs and formulate care plans.
- Excellent communication and interpersonal skills.
