Occupational Therapist

We are seeking a skilled Occupational Therapist to join our Community Advantage Rehabilitation team in Durham Region. As a Community Occupational Therapist, you will provide ministry-funded, community-based Occupational Therapy services to adults in their place of residence.
Key Responsibilities:
- Provide occupational therapy assessment and intervention in the areas of home safety, home modifications, equipment prescription, fall prevention, mobility, cognition, and stroke.
- Develop and implement best practice and evidence-based occupational therapy intervention in collaboration with clients/caregivers.
- Provide education to clients/caregivers and link to community agencies/resources when appropriate.
- Complete funding applications as applicable.
- Complete and maintain documentation related to assessment findings, care plans, progress, and discharge summaries according to CAR, the referral source, and COTO standards.
Requirements:
- Registered Occupational Therapist with the College of Occupational Therapists of Ontario.
- Experience in working with in a community setting is preferred but will consider new graduates.
- Ability to work independently and as part of an inter-professional team.
- Ability to prioritize, manage self-scheduling, and meet reporting deadlines.
- Excellent oral, written skills, and report writing.
- Proficiency with computers.
- Strong interpersonal and organizational skills.
- Reliable transportation is required for this position.
