Occupational Therapist

We are seeking a skilled Occupational Therapist to join our team at Community Advantage Rehabilitation, a division of Lifemark. As an Occupational Therapist, you will work with adults in the community to provide ministry-funded services and help them achieve their goals.
Key Responsibilities:- Provide occupational therapy services in the client's place of residence
- Conduct assessments and develop care plans to address home safety, mobility, cognition, and other areas
- Implement evidence-based interventions and provide education to clients and caregivers
- Collaborate with community agencies and resources to support clients
- Maintain accurate documentation and complete funding applications as needed
- Registered Occupational Therapist with the College of Occupational Therapists of Ontario
- Experience working in a community setting (preferable but not required)
- Ability to work independently and as part of an inter-professional team
- Excellent communication and organizational skills
- Reliable transportation required
- A flexible work schedule and independent management of caseload volume
