Occupational Therapist

We are seeking a highly skilled and motivated Occupational Therapist to join our team at Community Advantage Rehabilitation, a division of Lifemark. As an Occupational Therapist, you will play a vital role in providing community-based rehabilitation services to adults in Durham Region.
Job Summary:The successful candidate will be responsible for providing ministry-funded, community-based Occupational Therapy services to clients in their place of residence. This will involve conducting assessments, developing and implementing best practice and evidence-based occupational therapy interventions, and providing education to clients and caregivers.
Key Responsibilities:- Conduct assessments and develop care plans for clients
- Provide occupational therapy interventions in the client's place of residence
- Collaborate with clients, caregivers, and other healthcare professionals to achieve optimal outcomes
- Complete funding applications and maintain documentation as required
- Registered Occupational Therapist with the College of Occupational Therapists of Ontario
- Experience working in a community setting is preferred, but new graduates will be considered
- Ability to work independently and as part of an inter-professional team
- Excellent communication and interpersonal skills
- Proficiency with computers and ability to prioritize and manage self-scheduling
- A flexible work schedule and independent management of caseload volume
