Salary: The salary range for this position is approximately $35.87 - $43.82 per hour, based on experience and qualifications.

About the Role:

The Nursing Transition Coordinator plays a crucial role in supporting patients through the transition process, ensuring a smooth transfer from hospital to community care. As a member of the multidisciplinary team, you will work closely with health professionals to assess patients' needs, develop individualized care plans, and coordinate service delivery across programs and settings.

Key Responsibilities:

Conduct comprehensive electronic needs assessments to determine priority and urgencyDevelop and implement effective service plans to meet patients' needsCoordinate service delivery across programs and settings, liaising with healthcare providers and community resources as neededProvide education and support to patients, families, and caregivers, promoting independence and self-managementParticipate in team meetings, conferences, and professional development activities to stay up-to-date with best practices and share knowledge with colleagues

Requirements:

To be successful in this role, you will require:

  • Active registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
  • A minimum of five years facility-based nursing experience, including geriatric experience within the last three years
  • A minimum of two years of experience working in the community
  • Strong communication, conflict resolution, and time management skills
  • Ability to work independently and as part of a team, with strong problem-solving and critical thinking skills
