Are you a skilled nurse looking to make a difference in the lives of individuals with Cystic Fibrosis (CF)? We are seeking an experienced Registered Nurse to join our team as a Nurse Care Coordinator at the Kaye Edmonton Clinic, part of Alberta Health Services.
Job SummaryThis is a challenging and rewarding role that requires a unique blend of clinical expertise, communication skills, and compassion. As a Nurse Care Coordinator, you will be responsible for coordinating patient care, working closely with multidisciplinary teams, and advocating for the needs of our patients with CF.
About the Role- Assess, plan, implement, evaluate, and educate clients with CF in collaboration with clinic physicians, nurse practitioners, and other healthcare professionals.
- Apply nursing processes, critical thinking, problem-solving, and decision-making skills to assess, triage, and coordinate patient referrals for assessment and treatment of CF.
- Maintain knowledge of safe, accurate treatment options, medications, and care options for CF patients while ensuring complete and accurate documentation.
- Develop and implement patient-centered plans of care in collaboration with patients, families, and interdisciplinary healthcare teams to ensure continuity of care.
- Completion of an accredited nursing education program and active registration with the College of Registered Nurses of Alberta (CRNA).
- Minimum 5 years' experience in an acute care setting with extensive pulmonary knowledge, preferably in ambulatory clinics or CF case management.
- Excellent communication and documentation skills, with experience working with databases, electronic health records, and Microsoft Office applications.
- A competitive salary of $85,000-$110,000 per year, depending on experience.
