Salary

The estimated salary for this position is between $35.87 and $43.82 per hour, depending on experience.

About the Job

This Home Healthcare Professional will provide professional support services to clients of all ages and stages of life in their homes. The successful candidate will work collaboratively with the healthcare team, assessing needs, delivering therapeutic nursing services, coordinating resources, establishing goals, and evaluating outcomes.

The ideal candidate will have excellent communication and assessment skills, with the ability to work effectively in a fast-paced environment. A strong understanding of Microsoft Office and other computer programs is essential for this role.

Responsibilities

  • Provide case management and direct client care that meets established clinical standards.
  • Act as a consultant on the team, providing clinical expertise and direction to other team members regarding client cases.
  • Develop care plans based on unmet needs, using internal and external resources.
  • Facilitate complex discharges from Acute Care to the Home, working collaboratively with Transition Coordinators and multidisciplinary teams.
  • Participate in staff development and mentor student clinical experiences.

Requirements

  • Completion of an accredited nursing education program.
  • Active or eligible registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
  • Two years of recent nursing experience, obtained within the last 5 years, or completion of a refresher program within 2 years.
  • Self-directed and able to work independently, using sound professional judgment.

Bonus Points

  • Palliative care experience.
  • Previous Continuing Care and/or geriatric experience.
  • Experience with Meditech, Connect Care, and the RAI Home Care assessment tool.

Location

This position is located in Edmonton Zone, serving the Home Living area of Continuing Care.
