Nurse Care Coordinator for Home Dialysis

At Alberta Health Services, we are seeking a skilled Nurse Care Coordinator to join our multidisciplinary team in the Home Peritoneal Dialysis program.
Job Summary:
The successful candidate will be responsible for assessing new patients, planning and providing follow-up care for home dialysis patients. This position is based at West Edmonton Kidney Care.
About the Role:
As a Registered Nurse (RN), you will provide a wide range of nursing services to patients, families, communities, and populations, ensuring their safety and well-being. You will utilize nursing processes through critical thinking, problem-solving, and decision-making, teaching, counseling, and advocating on behalf of patients and their families.
Key Responsibilities:
Assess new patients and develop individualized care plansProvide education and training to patients and caregivers on dialysis replacement therapyCollaborate with multidisciplinary teams to ensure comprehensive patient careMonitor patient progress and adjust care plans as neededRequirements:
To be successful in this role, you will require:
- A degree in nursing from an accredited institution
- Active registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification
- Prior experience in community nursing or nephrology care is an asset
What We Offer:
We offer a competitive salary range of $83,000 - $123,000 per year, depending on experience, as well as opportunities for professional growth and development within Alberta Health Services.
