Clinical Health Navigator for Winnipeg Health Region

Salary: $40.62 - $49.83 per hour, based on experience.
As a Clinical Health Navigator, you will play a vital role in providing telephone-based health information and referral assistance services to individuals in need. Your professional assessment and decision-making skills will help callers navigate the healthcare system, ensuring they receive the right care at the right time.
Key Responsibilities:
- Provide professional assessment and guidance to callers, using your expertise in health care to make informed decisions.
- Use approved protocols and the nursing process to deliver high-quality service.
- Document caller data and encounters on the electronic health record.
- Practice client empowerment and health promotion principles.
Requirements:
- Minimum 3-5 years of experience in an Emergency Department or Community Health setting.
- Prior experience with phone line, crisis intervention, and Windows or Outlook preferred.
- Graduate of an approved Registered Nursing educational program.
Preferred Qualifications:
- Fluency in English required, with French and other languages an asset.
- Demonstrated advanced triage or assessment skills.
- Excellent communication and keyboard skills.
