We are a leading provider of home care services in Canada, committed to delivering high-quality care to our clients and their families. We are seeking a skilled Client Service Coordinator to join our team in Downtown Toronto.

Job Summary

The Client Service Coordinator will work collaboratively with our community PSW and Nurses to schedule and coordinate PSW and Nurse services. This role requires strong communication and problem-solving skills, as well as the ability to multitask and prioritize.

Key Responsibilities

  • Works collaboratively with community PSW and Nurses to schedule and coordinate PSW and Nurse services
  • Liaises with clients, staff, and community partners to ensure seamless care delivery
  • Acts as a problem solver for clients and responds to inquiries from LHIN and employees
  • Reports client and staff concerns to Home Support Supervisor
  • Liaises with LHIN representatives regarding client care and collects pertinent client data into the computer system
  • Provides administrative support to the personal support worker and Nurse program

Requirements

  • Minimum of 2 years working in an office setting (healthcare setting preferred)
  • High school diploma or equivalent
  • Medical Office Admin Certificate/Diploma an asset
  • Medical terminology knowledge an asset but not required
  • Ability to handle a high volume of calls while delivering exceptional customer service (healthcare industry experience an asset)
  • Ability to multitask and prioritize
  • Excellent English written and verbal communication skills (Additional language skills considered an asset)
  • Excellent customer service and organizational skills
  • Proficient in Word, Outlook and ability to learn proprietary software
