Job Summary

We are seeking a highly skilled and compassionate Indigenous Care Coordinator to join our Mental Health and Substance Use (MHSU) department at Northern Health Authority. As a key member of our team, you will play a vital role in providing culturally safe care and services to Indigenous patients and families.

Key Responsibilities
  • Contribute to the clinical design, implementation, and monitoring of individual Indigenous patient care plans to maximize community support and cultural safety.
  • Work collaboratively with a multi-disciplinary care team to provide services to Indigenous, Métis, and Inuit individuals throughout their engagement with acute and/or primary care services.
  • Facilitate education and learning of the care team on Indigenous approaches to wellness, trauma-informed care, cultural safety, and long-term discharge planning goals.
  • Collaborate with healthcare teams in First Nations communities and Métis Chartered communities to ensure coordinated care between Northern Health services and community services.
  • Work closely with the Indigenous Patient Service Assistant to identify and meet the needs of Indigenous patients and families.

Requirements
  • Baccalaureate degree in Social Sciences, Allied Health, or a related healthcare discipline, plus two years of experience working in a healthcare setting, or an equivalent combination of education, training, and experience.
  • Knowledge and appreciation of the unique history, cultures, and rights of First Nations, Métis, and Inuit Peoples in Canada.
  • Lived experience with Indigenous, Métis, and Inuit populations and demonstrated extensive knowledge of First Nations, Métis, and Inuit healthcare needs, services, and issues, as well as knowledge of culture, protocols, traditions, and ideology of Indigenous people and organizations in northern British Columbia.
  • Preference will be given to First Nations, Métis, and Inuit candidates.
Skills and Abilities
  • Ability to complete initial and ongoing client assessments through appropriate technical, therapeutic, safety-type interventions.
  • Ability to integrate best practice and current research evidence to support professional practice decisions and actions.
