Indigenous Care Coordinator

We are seeking a highly skilled and compassionate Indigenous Care Coordinator to join our team at Northern Health Authority. As a key member of our Mental Health & Substance Use department, you will play a vital role in providing culturally safe care to Indigenous patients and families.
About the Role:This is an exciting opportunity to bring your expertise and passion for Indigenous health to a dynamic and growing team. As an Indigenous Care Coordinator, you will work closely with healthcare teams to design, implement, and monitor individual patient care plans that prioritize community support, cultural safety, and respectful care.
Key Responsibilities:- Collaborate with healthcare teams to develop and implement patient care plans that address the unique needs of Indigenous patients and families.
- Provide education and training to healthcare teams on Indigenous approaches to wellness, trauma-informed care, and cultural safety.
- Work closely with Indigenous Patient Service Assistants to identify and meet the needs of Indigenous patients and families.
- Develop and maintain relationships with First Nations communities and Métis Chartered communities to ensure coordinated care.

Requirements:To be successful in this role, you will require a baccalaureate degree in Social Sciences, Allied Health, or a related healthcare discipline, plus two years of experience working in a healthcare setting. You will also need to demonstrate knowledge and appreciation of the unique history, cultures, and rights of First Nations, Métis, and Inuit Peoples in Canada.
Preferred Qualifications:Preference will be given to First Nations, Métis, and Inuit candidates who have lived experience working with Indigenous populations and have demonstrated extensive knowledge of First Nations, Métis, and Inuit health care needs, services, and issues.
