Indigenous Care Coordinator Position

We are seeking a highly skilled and compassionate Indigenous Care Coordinator to join our Mental Health and Substance Use department at Northern Health Authority. As a key member of our care team, you will play a vital role in providing culturally safe and respectful care to Indigenous patients and families.
Under the direction of the Team Lead, you will contribute to the clinical design, implementation, and monitoring of individual Indigenous patient care plans. Your expertise will help maximize community support, cultural safety, and respectful care for Indigenous patients and families. You will work closely with our multi-disciplinary care team to provide services to Indigenous, Métis, and Inuit individuals throughout their engagement with acute and/or primary care services.
As an Indigenous Care Coordinator, you will facilitate the education and learning of the care team in areas such as understanding the services and supports available in local Indigenous communities, Indigenous approaches to wellness, trauma-informed care, cultural safety, and long-term discharge planning goals. You will collaborate with healthcare teams in First Nations communities and Métis Chartered communities to ensure that patient care is coordinated between Northern Health services and community services.

What We Offer

Qualifications
We are looking for a candidate with a Baccalaureate degree in Social Sciences, Allied Health, or a related healthcare discipline, plus two years of experience working in a healthcare setting, or an equivalent combination of education, training, and experience. Knowledge and appreciation of the unique history, cultures, and rights of First Nations, Métis, and Inuit Peoples in Canada is required. Lived experience with Indigenous, Métis, and Inuit populations and demonstrated extensive knowledge of First Nations, Métis, and Inuit healthcare needs, services, and issues, as well as knowledge of culture, protocols, traditions, and ideology of Indigenous people and organizations in northern British Columbia, is an asset. Preference will be given to First Nations, Métis, and Inuit candidates.
Skills and Abilities
We are looking for a candidate with the ability to complete initial and ongoing client assessments through appropriate technical, therapeutic, safety-type interventions. The ideal candidate will also have the ability to integrate best practice and current research evidence to support professional practice decisions and actions. Additionally, the candidate should be able to communicate effectively with clients, families, the public, medical staff, and members of the interdisciplinary team using verbal, written, computer communication means. The ability to incorporate cultural safety into all aspects of practice, develop knowledge and understanding of local Indigenous culture and traditions, and develop cross-sector collaborative relationships in the interests of providing culturally safe care and services is also essential. The candidate should be able to apply conflict resolution skills, promote client-focused care for and with clients and significant others, sensitivity to diverse cultures and preferences, client advocacy, and social justice concerns. The ability to organize work, set objectives, and establish priorities, foster teamwork and a commitment to excellence in the provision of client care, and operate related equipment is also required. Finally, the candidate should be able to demonstrate confidence in knowledge-based problem-solving, physical ability to carry out the duties of the position, and flexibility and openness with all aspects of programming.
