Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Island Community Mental Health. As a LPN, you will play a vital role in providing high-quality care to our clients and contributing to their recovery journey.

Job Summary

The successful candidate will be responsible for assessing, planning, implementing, and evaluating nursing care to meet the needs of clients in a licensed care mental health setting. You will work collaboratively with an interdisciplinary team to develop, implement, and assess client care plans, and provide clinical input to ensure the best possible outcomes for our clients.

Key Responsibilities
  • Assess clients' physical and emotional needs, and develop individualized care plans to meet their unique requirements.
  • Administer medications, provide medication reminders, and maintain accurate records of medication administration.
  • Monitor clients' safety and well-being, and report any concerns or incidents to leadership.
  • Collaborate with the interdisciplinary team to develop and implement client care plans, and provide clinical input to ensure the best possible outcomes.
  • Provide support and education to clients and their families, and promote a culture of recovery and wellness.
Requirements
  • Graduation from a recognized program for Practical Nurses.
  • Current full practicing license with the College of Licensed Practical Nurses of BC (CLPNBC) in good standing.
  • Certificates in CPR and First Aid, preferred.
  • Class 5 Driver's License, ASIST, Violence Prevention, Motivational Interviewing, and Harm Reduction an asset.
