Job Description:

As a Licensed Practical Nurse - Care Coordinator at Baptist Housing, you will be responsible for overseeing medication administration, wound care, and resident assessments. Your role will involve utilizing a comprehensive care model to improve residents' lives and maintain their independence. Working in a collaborative environment, you will contribute to resident-centered care planning, evaluate service delivery, and collaborate with Registered Nurses for complex cases. Your communication skills and compassionate approach to care will ensure residents feel valued and heard. Additionally, you will maintain accurate resident information using advanced systems and participate in wellness programs to create a positive environment for residents and team members.

Key Responsibilities:

  • Develop and implement individualized care plans to meet residents' unique needs
  • Collaborate with interdisciplinary teams to ensure seamless care delivery
  • Conduct regular resident assessments to identify areas for improvement
  • Provide education and support to residents and their families on health-related topics
  • Participate in quality improvement initiatives to enhance resident care and satisfaction

Requirements:

  • Licensed Practical Nurse Diploma from a recognized institution
  • Member in good standing with British Columbia College of Nurses & Midwives
  • Current First Aid and Food Safe Certificates
