About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Island Community Mental Health. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to individuals experiencing mental health challenges.

Key Responsibilities
  • Assessment and Care Planning: Conduct thorough assessments of clients' needs and develop personalized care plans to meet their unique requirements.
  • Medication Management: Administer medications safely and accurately, and provide medication reminders as needed.
  • Client Support: Encourage and assist clients with activities of daily living, such as bathing, skin care, and oral hygiene.
  • Communication and Collaboration: Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date records of client progress, including progress notes, observations, and care plans.
Requirements
  • Education and Training: Graduation from a recognized program for Practical Nurses.
  • Licensure and Certification: Current full practicing license with the College of Licensed Practical Nurses of BC (CLPNBC) in good standing, and certificates in CPR and First Aid (preferred).
  • Skills and Abilities: Demonstrated awareness of own cultural values and biases, diversity, and individual beliefs and experiences; promotes application of recovery-oriented PSR in practice.
