Palliative Care Nurse

About this opportunity
We are seeking a skilled Registered Nurse to join our Palliative Care team. As a Palliative Registered Nurse, you will have scheduled visits to care for patients in various settings, working with patients of all ages and complexity of care needs.
Key responsibilities
- Using appropriate assessment tools and techniques to assess patients' individualized needs
- Executing and continuously monitoring care plans
- Engaging patients in a collaborative approach by building rapport and trust
- Maintaining and updating professional patient records related to patient encounters and plans
What we're looking for
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle
- Palliative care experience is mandatory
- Post-graduate education in Palliative care is strongly preferred
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
