Job Description

This role involves providing palliative care to patients in various settings, working with patients of all ages and complexities. You will have scheduled visits to care for patients in their homes, playing a key role in helping them remain comfortable.

The working hours are 8am to 4pm or 2pm to 10pm, with every third weekend included. Autonomy, independence, and a work-life balance make this job attractive.

Your Responsibilities:
  • Assess patients' individualized needs using appropriate tools and techniques
  • Execute and continuously monitor care plans
  • Engage patients using a collaborative approach by building rapport and trust
  • Maintain and update professional patient records related to patient encounters and plans
Requirements:
  • Registration with the CNO as a Registered Nurse in good standing
  • Valid driver's license and access to a vehicle is required
  • Palliative care experience is mandatory
  • Post-graduate education in Palliative care is strongly preferred
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Passion for making a difference in your community
  • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
Compensation:

RN Visit Rate + Palliative Premium; As per collective agreement
