Palliative Care Nurse

About the Role:
We are seeking a compassionate and skilled Registered Practical Nurse to join our Palliative Care team. As a Palliative Registered Practical Nurse, you will provide high-quality care to patients in their homes, working collaboratively with RNs to support complex patients.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice.
- Provide safe and ethical nursing care, using appropriate assessment tools and techniques to assess patients' individual needs.
- Execute and continuously monitor care plans, adjusting as required to meet patient needs.
- Engage patients using a collaborative approach, building rapport and trust to promote positive outcomes.
- Maintain and update professional patient records related to patient encounters and plans.
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- Palliative care experience is mandatory.
- Post-graduate education in Palliative care is strongly preferred.
- Valid driver's license and access to a vehicle is required.
- Experience in delivering community care is an asset.
- Reliability and flexibility.
- Willingness to travel to and from patient residences within a designated area.
- Experience working with vulnerable populations.
- Passion for making a difference in your community.
