Job DescriptionThis part-time Registered Practical Nurse (RPN) or Registered Nurse (RN) position involves working in a dynamic and supportive team environment. Key responsibilities include:
- Conducting comprehensive client assessments and developing personalized care plans
- Implementing and evaluating care based on best evidence and practices, while considering the unique needs of each client
- Providing primary care lead services, collaborating with other care team members and support functions
- Supporting clients through care transitions by connecting them with community resources and services
To succeed in this role, you will require:
- A valid driver's license and vehicle for travel
- A Canadian Nurses Association (CNA) specialty certification (asset)
- Enthusiasm for community care and passion for evidence-informed practice and excellence in care

This Nursing position is part of the Community Commitment Program for Nurses (CCPN), which provides a $25,000 incentive to eligible nurses in exchange for a 24-month commitment to an eligible employer. To be eligible, you must commit to 24 months of employment at a full-time work schedule offered by the employer, without simultaneously receiving funds from the Ministry of Health for the Tuition Support Program for Nurses.
