Home and Community Care Director

VON Canada (Ontario) is seeking a seasoned healthcare professional to fill the role of Manager Home and Community Care. As a Registered Nurse, you will be responsible for providing leadership and oversight to staff, ensuring the delivery of safe and effective client care within your program area.
Job SummaryThe Manager Home and Community Care - Registered Nurse provides professional practice leadership to staff and is accountable for working as part of an integrated Regional management team to ensure the successful execution of site operational goals. Key responsibilities include directing and monitoring delivery of client care services, assisting staff with risk assessments and investigations, and contributing to quality improvement activities.
About This RoleThis leadership position requires a strong understanding of current regulatory guidelines and VON policies, procedures, and standards. You will work closely with internal stakeholders to develop team procedures and guidelines to ensure best practices are met in the completion of all tasks or programs. Effective communication and collaboration skills are essential for success in this role, as you will interact regularly with clients, families, and community organizations.
Key Responsibilities- Directs and monitors delivery of client care services and day-to-day functions and performance of front-line staff in accordance with current regulatory guidelines and VON policies, procedures, and standards.
- Assists staff to complete risk assessments, investigations of issues, injuries, and reporting related to client or employee incidents.
- Monitors performance related to funder expectations regularly and contributes to the development and implementation of quality improvement activities to achieve performance targets through monitoring of reported services.

Education, Designations, and ExperienceTo be considered for this role, you must possess a Post-Secondary education in a related field, certification as a Registered Nurse, and a minimum of 5 years' experience in a functional area similar to this position. Additionally, you should have a minimum of 2 years' experience managing others, experience working in a unionized environment, and excellent critical thinking, assessment, and problem-solving skills.
