Due to our ongoing expansion, we are seeking to enhance our team of Therapists within the Addiction Medicine Program. As a Mental Health and Addiction Therapist, you will play a crucial role in a collaborative, interdisciplinary team that focuses on providing integrated services and programs to our clients. Your responsibilities will include treatment planning, clinical case coordination, and delivering direct services through both individual and group sessions. This position is ideal for Mental Health Therapists, Psychotherapists, or Social Workers with experience in psychotherapy, who thrive in a dynamic and fast-paced environment, assisting individuals facing complex mental health and addiction challenges within a concurrent disorders framework.

Key Responsibilities:
  • Conduct assessments to inform treatment planning and guide interventions.
  • Deliver case management services, including service planning, coordination, navigation, and progress evaluation.
  • Facilitate individual and group psychotherapy using evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, and Cognitive Behavioral Therapy.
  • Provide support to family members and lead educational group sessions.
  • Collaborate closely with Physicians and the clinical team to monitor patient experiences and adjust treatment plans as necessary.
  • Oversee discharge planning and referrals to appropriate community resources.
  • Identify discharge needs based on comprehensive assessments and care plans.
  • Establish treatment goals in collaboration with patients and team members, developing actionable steps to achieve these goals.
  • Lead team evaluations of treatment progress and outcomes.
  • Ensure effective communication among patients, team members, and external stakeholders.
  • Maintain accurate documentation and record-keeping in accordance with organizational policies.
Qualifications:
  • Bachelor's or Master's degree in a recognized Social Science program with specialized education in addiction.
  • Membership in good standing with the relevant regulatory body in Ontario or in pursuit of appropriate credentials.
  • Proficiency in delivering CBT, DBT, and other evidence-based approaches for complex mental health and substance use disorders.
  • Demonstrated experience in assessment and treatment using evidence-based methodologies.
  • Strong communication and interpersonal skills.
  • Ability to lead and facilitate an interdisciplinary team effectively.
  • Experience in individual and group counseling, including the delivery of psycho-educational content.
  • Capacity to implement inclusive and culturally responsive practices.
  • Knowledge of addiction issues and concurrent care pathways.
  • Valid Ontario driver's license and access to a vehicle as needed.
  • Familiarity with relevant legislation, including the Mental Health Act and Health Care Consent Act.
