Key Responsibilities- Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards.
- Develop and implement client-centered care plans in collaboration with the client's integrated care team.
- Ensure coordinated delivery of care through effective communication with all members of the client's care team.
- Document all assessment findings, treatment plans, progress, and discharge summaries of interventions as required by the profession, referral source, and organization.
- Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
- Master's degree in Occupational Therapy from a recognized school of Occupational Therapy.
- Member in good standing with the College of Occupational Therapists of Ontario.
- Valid Driver's License with access to a vehicle.
- Compassionate and empathetic individual with excellent communication skills.
- Ability to work independently and as part of a team.
- Skilled in caseload management and electronic documentation.
- Able to provide virtual care and possess a registered ADP authorizer (asset).
