Job Title: Outreach Worker Forensic Psychiatric Services

BC Mental Health & Substance Use Services is seeking a highly skilled and compassionate Outreach Worker to join our Forensic Psychiatric Services team in Surrey, BC.

Job Summary:

We are looking for a dedicated and experienced Outreach Worker to provide support and services to individuals with complex mental health and substance use challenges. As an Outreach Worker, you will work collaboratively with our multidisciplinary team to provide outreach services, including monitoring, screening, and interviewing clients to obtain information regarding their health condition and environment.

Key Responsibilities:
  • Monitor, screen, and interview clients to obtain information regarding their health condition and environment.
  • Report significant and unsafe behavior, physical and cognitive changes, in accordance with standard procedures or specific directions.
  • Arrange, plan, escort, and accompany clients on medical appointments and errands, while providing emotional, moral support, and assistance.
  • Create linkages to other community resources and programs, contact relevant organizations, and assist clients with processing forms.
  • Act as a liaison between various agencies, providing information on services and supporting the development of self-efficacy and self-care.
Requirements:
  • Graduation from a recognized Social Services Worker certificate program or an equivalent combination of education, training, and experience.
  • Valid Class 5 vehicle license.
  • Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action (2015), In Plain Sight Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and other related reports.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
