Job Title: Forensic Community Liaison Nurse

BC Mental Health & Substance Use Services is seeking a highly skilled and compassionate Forensic Community Liaison Nurse to join our team. As a key member of our multidisciplinary team, you will work closely with individuals with severe and persistent psychiatric illness, providing expert nursing care and support.

Key Responsibilities:
  • Conduct comprehensive biopsychosocial assessments, including mental status examinations and physical health evaluations, to identify client needs and develop individualized treatment plans.
  • Collaborate with mental health teams, private psychiatrists, and other healthcare professionals to ensure seamless care coordination and continuity.
  • Provide community case coordination and advocacy services, connecting clients with essential resources, such as housing, medical care, and social services.
  • Maintain accurate and concise documentation of client information, adhering to regulatory requirements and professional standards.
Requirements:
  • Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses & Midwives (BCCNM).
  • Two (2) years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
  • Valid BC Drivers License.
