About the Role

Vancouver Coastal Health is seeking a skilled Community Liaison Worker to join our Integrated Care Team in Sechelt, BC. As a key member of our team, you will provide essential support services to clients with mental health and substance use issues.

Key Responsibilities
  • Provide holistic support services to clients, including mental health and substance use support.
  • Work collaboratively with interdisciplinary care teams to coordinate services and ensure comprehensive care.
  • Monitor client progress, document observations, and provide verbal reports as required.
  • Facilitate access to community resources and provide information on available services.
  • Liaise with mental health, addictions, and medical professionals to ensure seamless care.
  • Plan, organize, and facilitate groups as needed, and assist clients with daily living activities.
  • Act as an advocate for clients and provide support related to complex healthcare needs.
  • Establish and maintain therapeutic relationships based on respect and trust.

Qualifications
  • Certificate from a recognized Health Care Worker program, plus two years' recent experience in a related field.
  • Valid B.C. Driver's license and access to a personal vehicle for business purposes.
