Key Responsibilities

  • Provide holistic support services to clients, addressing their mental health, substance misuse, and primary care needs.
  • Work collaboratively with multiple stakeholders, including mental health professionals, social workers, and community service providers.
  • Monitor client progress, document observations, and provide verbal reports as required.
  • Facilitate access to community resources and services, providing information and support to clients.
  • Liaise with mental health, addictions, and medical professionals to ensure comprehensive care.
  • Plan, organize, and facilitate groups, as well as participate in case planning and client advocacy.
  • Establish and maintain therapeutic relationships with clients, based on respect and empathy.
