About the Role
Vancouver Coastal Health is seeking a skilled Community Liaison Specialist to join our Integrated Care Team. As a key member of our team, you will play a vital role in providing support services to clients with mental health and substance misuse issues.
Key Responsibilities
- Provide holistic support services to clients, addressing their mental health, substance misuse, and primary care needs.
- Work collaboratively with multiple stakeholders, including mental health professionals, social workers, and community service providers.
- Monitor client progress, document observations, and provide verbal reports as required.
- Facilitate access to community resources and services, providing information and support to clients.
- Liaise with mental health, addictions, and medical professionals to ensure comprehensive care.
- Plan, organize, and facilitate groups, as well as participate in case planning and client advocacy.
- Establish and maintain therapeutic relationships with clients, based on respect and empathy.
