Position Overview

Become a Community Health Worker with Vancouver Coastal Health (VCH)

Community Health Workers play a vital role in our communities, providing essential care that significantly enhances the quality of life for clients and patients.

As a Community Care Assistant at Vancouver Coastal Health, your responsibilities will include:

  • Delivering home support services to clients under the guidance of a Supervisor/Coordinator/Manager or their delegate.
  • Assisting clients with varying levels of care while observing and reporting any notable changes in their condition.
  • Working collaboratively with the healthcare team, clients, and their families to promote independence in daily living activities.
  • Encouraging and motivating clients to engage in their own care.
  • Providing valuable input and feedback during the care planning process with the healthcare team.

New graduates from the Health Care Assistant program are encouraged to apply.

Community Health Workers at Vancouver Coastal Health operate within a client and family-centered care framework, adhering to the organization's established vision and values.

Requirements

Education & Experience

  • Completion of a recognized Health Care Assistant Program.
  • Current registration with the BC Community Health Worker and Care Aide Registry.
  • Experience working with individuals with dementia, geriatric, or palliative care needs is advantageous.
  • A valid BC Driver's license and access to a vehicle are required for local travel.

Knowledge & Skills

  • Understanding of client-centered care and promoting independence.
  • Knowledge of proper body mechanics and injury prevention techniques.
  • Ability to operate mechanical lifts while adhering to safety protocols.
  • Familiarity with workplace violence prevention and personal safety methods.
  • Experience in patient-centered care, particularly with individuals with dementia.
  • Ability to apply a harm reduction approach in practice.
  • Basic technological proficiency, including the use of computerized client care systems and mobile devices.
  • Effective communication skills, both verbal and written, across various platforms (e.g., electronic records, phone, in-person).
  • Understanding of self-care and motivational support strategies.
  • Ability to provide culturally safe and competent care, especially when working with Indigenous populations.
  • Capability to prioritize tasks effectively within a team setting.
  • Willingness to take direction from the healthcare team and clients.
  • Skills in home management tasks (e.g., cleaning, meal preparation).
  • Ability to inspire clients and support their progress towards therapeutic goals.
  • Capacity to respond appropriately in a professional environment.
  • Physical ability to fulfill the responsibilities of the role.
Additional Information

Work hours, including days off and work locations, may vary based on operational needs and collective agreements. Successful candidates may need to undergo a Criminal Records Review Check. In compliance with current Public Health Orders, all employees at Vancouver Coastal Health must be fully vaccinated against COVID-19 or have received the most recent updated vaccine. Proof of vaccination will be required.
