LPN - Senior Care Specialist

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team as a Senior Care Specialist. In this role, you will play a vital part in delivering high-quality care to our residents, promoting their overall well-being and quality of life.
Responsibilities:- Conduct thorough assessments of residents' needs and develop personalized care plans to ensure their physical, emotional, and social well-being.
- Collaborate with the Resident Care Director to evaluate and document residents' health status, identifying areas for improvement and implementing changes as needed.
- Provide regular wellness visits to residents, monitoring their health and addressing any concerns or issues that arise.
- Develop and maintain effective communication with residents, their families, and healthcare providers to ensure seamless care coordination.
- Stay up-to-date with the latest clinical practices and regulations, ensuring compliance with Sunrise Senior Living's standards and federal, state, and local regulations.
- Graduate of an approved college or school of nursing with a current state license as a Licensed Practical Nurse.
- Minimum of one year of experience in a healthcare setting, preferably in senior care or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Ability to work flexible hours, including weekends and evenings, to meet the needs of our residents and their families.
