At Sunrise Senior Living, we are committed to providing high-quality care and services to our residents. We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Senior Care Specialist.
Responsibilities:- Conduct regular wellness visits for residents to identify and address their needs.
- Collaborate with the Resident Care Director to develop and implement individualized care plans.
- Monitor and document resident vital signs, weights, and medication profiles.
- Provide clinical support and education to community team members.
- Ensure compliance with federal, state, and local regulations, as well as Sunrise policies and procedures.
- Graduate of an approved college or school of nursing.
- Current BCCNM registration and state license as a LPN/LVN.
- Minimum of one year experience in a long-term care environment.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
