Job Summary:The LPN - Wellness Nurse is responsible for promoting the overall well-being of our residents. This includes identifying ongoing needs and services, ensuring proper follow-through, and participating in resident evaluations and data gathering.
Key Responsibilities:- Conduct monthly wellness visits for all residents to assess their health and wellness needs.
- Monitor and follow up with residents who have experienced any falls, accidents, or incidents, documenting and reporting as required.
- Assist the Resident Care Director in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Ensure weights and vital signs are obtained monthly for each resident.
- Maintain communications with residents' families and responsible parties to ensure medical needs are being met.
- Collaborate with the Resident Care Director and community team to promote infection control standards, including resident testing, training, and skills checks.
- Regularly monitor each resident's medication and treatment profile to ensure accurate administration and documentation.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year of experience in home health, assisted living, hospital, or long-term care environment.
- Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
- Ability to handle multiple priorities, possess written and verbal skills for effective communication, and demonstrate good judgment, problem-solving, and decision-making skills.
