About the Role

Vision Loss Rehabilitation Canada is seeking a skilled Client Care Navigator to join our team. As a Client Care Navigator, you will play a critical role in coordinating and integrating supports and services around individuals living with vision loss.

Key Responsibilities
  • Coordinate and integrate supports and services around individuals living with vision loss
  • Assess client needs and develop care plans to achieve desired outcomes
  • Strengthen communication and linkages between Vision Loss Rehabilitation Canada staff and external community healthcare providers
  • Ensure timely access to Vision Loss Rehabilitation Canada service information and connection to the healthcare system
  • Monitor and provide proactive follow-up for clients to ensure ongoing collaboration across services/providers
  • Identify opportunities for new or emerging care options to meet changing needs and address service/support gaps
  • Document all service-related activities in the client health record following Vision Loss Rehabilitation Canada policies
Requirements
  • 3-5 years of client service experience in the health and/or social service sectors
  • Experience working directly with individuals living with vision loss
  • Knowledge of available community services/supports and clinical, social, and residential care options
  • Understanding of roles and linkages across primary care, community care, and specialized geriatric services
  • Strong knowledge of client-centered philosophy and clinical expertise related to client-centered interviewing, assessments, and scheduling practices
  • Experience working in settings requiring inter-professional collaboration
  • Excellent communication (verbal and written) and interpersonal skills, including shared decision-making and facilitation
  • Ability to prioritize workload and manage competing tasks
  • Initiative and resourcefulness, with excellent problem-solving skills
  • Proficiency in technology, including Microsoft Office and case management and care coordination systems
  • Demonstrated ability to work independently and within a team
  • Cultural sensitivity and diversity expertise
Preferred Qualifications
  • Registered health professional designation
  • Minimum bachelor's degree in Social Work, Gerontology, or other related healthcare discipline
  • 3 or more years of experience working with individuals with vision loss
