Job Description

Job Title: Client Care Navigator

Job Summary: We are seeking a highly skilled and compassionate Client Care Navigator to join our team at Vision Loss Rehabilitation Canada. As a key member of our team, you will play a vital role in coordinating and integrating supports and services for individuals living with vision loss.

Key Responsibilities:

  • Assess client needs and develop personalized care plans to achieve optimal outcomes.
  • Coordinate and schedule appointments with primary and specialized service providers.
  • Monitor client satisfaction and take remedial action as required.
  • Identify opportunities for new or emerging care options to meet changing needs and address service/support gaps.
  • Ensure accurate and timely documentation of client health records.
  • Build and maintain strong relationships with clients, family members, and caregivers.
  • Collaborate with professionals in other agencies and community groups to ensure seamless care coordination.
  • Participate in multi-disciplinary team meetings to share knowledge and best practices.
  • Proactively facilitate linkages, communication, and information exchange opportunities between clients and service providers.

Requirements:

  • 3-5 years of client service experience in the health and/or social service sectors.
  • Experience working directly with individuals living with vision loss.
  • Knowledge of available community services/supports and clinical, social, and residential care options.
  • Understanding of roles and linkages across primary care, community care, and specialized geriatric services.
  • Strong knowledge of client-centered philosophy and clinical expertise related to client-centered interviewing, assessments, and scheduling practices.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to prioritize workload and manage competing tasks.
  • Proficiency in technology, including Microsoft Office and case management systems.

Preferred Qualifications:

  • Registered health professional designation.
  • Minimum bachelor's degree in Social Work, Gerontology, or other related healthcare discipline.
  • 3 or more years of experience working with individuals with vision loss.
