Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in ensuring that our patients receive the highest quality care and support in the comfort of their own homes.
Key Responsibilities- Assess and Determine Patient Care Needs: Conduct thorough assessments of patients' care needs and determine eligibility for services.
- Provide Access and Referrals to Community Services: Link patients with community services and resources to support their care and well-being.
- Coordinate and Monitor Care Plan Delivery: Work with patients, caregivers, and healthcare providers to coordinate and monitor the delivery of care plans.
- Establish a Helping Relationship with Patients and Families: Build trust and rapport with patients and their families to ensure their needs are met and their voices are heard.
- Membership in Good Standing with Regulatory Body: Hold membership in good standing with the applicable regulatory body (e.g. College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
- 2+ Years of Recent Experience in Community Health: Possess 2+ years of recent experience in community health or a related field.
- Knowledge of Healthcare Delivery System and Community Resources: Have a strong understanding of the healthcare delivery system and community resources available to support patients.
- Excellent Interpersonal and Communication Skills: Possess excellent interpersonal and communication skills to work effectively with patients, caregivers, and healthcare providers.
- Experience Working with Diverse Patient Groups: Have experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
- Case Management Experience or Recent Related Community Experience: Possess case management experience or recent related community experience.
- Ability to Speak French or Another Second Language: Have the ability to speak French or another second language.
