Job Description

Job Title: Client Care Navigator

Job Summary: We are seeking a highly skilled and compassionate Client Care Navigator to join our team at Vision Loss Rehabilitation Canada. As a Client Care Navigator, you will play a critical role in coordinating and integrating supports and services around individuals living with vision loss.

Key Responsibilities:

  • Assess client needs and develop personalized care plans to achieve their goals
  • Coordinate and schedule appointments with primary and specialized service providers
  • Monitor and provide proactive follow-up to ensure ongoing collaboration across services/providers
  • Identify opportunities for new or emerging care options to meet changing needs and address service/support gaps
  • Document all service-related activities in the client health record
  • Interact with clients, their family, and caregivers to develop and identify vision-related goals
  • Build and maintain good working relationships with professionals in other agencies and community groups
  • Participate in multi-disciplinary team meetings
  • Facilitate linkages, communication, information exchange opportunities, and coordination between clients and service providers

Requirements:

  • 3-5 years of client service experience in the health and/or social service sectors
  • Experience working directly with people living with vision loss
  • Knowledge of available community services/supports and clinical, social, and residential care options
  • Understanding of roles and linkages across primary care, community care, and specialized geriatric services
  • Strong knowledge of client-centered philosophy
  • Excellent communication (verbal and written) and interpersonal skills
  • Ability to prioritize workload and manage competing tasks
  • Proficiency in technology (e.g., Microsoft office and case management and care coordination systems)

Preferred Qualifications:

  • Registered health professional designation
  • Minimum bachelor's degree in Social Work, Gerontology, or other related healthcare discipline
  • 3 or more years' experience working with individuals with vision loss
