About the Role

We are seeking a skilled Registered Practical Nurse (RPN) to join our team in providing high-quality community care to patients in their homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities

  • Participate in scheduled visits to assess and address the needs of patients in their homes
  • Provide safe and effective nursing care, using appropriate assessment tools and techniques
  • Develop and implement individualized care plans, in collaboration with patients and their families
  • Engage patients in their care, using a collaborative approach to build trust and rapport
  • Maintain accurate and up-to-date patient records, reflecting patient encounters and care plans

Requirements

  • Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a reliable vehicle
  • Experience in delivering community care, an asset
  • Reliability, flexibility, and a willingness to travel to patient residences
  • Passion for making a difference in the community
