- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Provide safe and ethical nursing care to patients of all ages
- Use assessment tools and techniques to identify patient needs and develop care plans
- Collaborate with patients and their families to build rapport and trust
- Maintain accurate and up-to-date patient records
Requirements
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care is an asset
- Reliability and flexibility
- Ability to travel to and from patient residences within a designated area
