Position Overview

VCH is seeking a Nurse to become a vital member of the team at Three Bridges Community Health Centre, focusing on the STOP HIV/AIDS Initiatives. We welcome your application to join our mission.

About the STOP HIV/AIDS Initiatives:

The STOP HIV Outreach Team consists of a dedicated group of nurses, social workers, and outreach professionals who provide intensive case management for individuals living with HIV who face challenges in accessing care and adhering to treatment plans. Our goal is to transition clients to lower-intensity resources as they develop greater self-care capabilities. The support we offer is tailored to each client's unique care plan.

Responsibilities of the HIV/AIDS Community Health Nurse include:

Collaborating within a multidisciplinary team to deliver outreach services to at-risk and marginalized populations. Offering frontline primary health care services, including immediate assessment, intervention, and treatment. Conducting pre- and post-test counseling for HIV antibody testing, providing results, and ongoing support to address the physical and psychosocial needs of clients. Delivering harm reduction education and counseling related to Sexually Transmitted Infections (STIs) and HIV in diverse environments. Distributing harm reduction supplies to those in need. Partnering with community organizations to reach populations at heightened risk for STDs and HIV. Coordinating with AIDS organizations to develop and implement educational workshops and programs targeting specific groups. Assessing health issues, behaviors, needs, and barriers faced by at-risk populations. Engaging in collaborative team efforts to devise innovative strategies for addressing challenges and participating in research initiatives as required.

Qualifications

Education & Experience

Active registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). A minimum of two (2) years of recent relevant clinical nursing experience in community-based or Indigenous-led organizations, focusing on screening, assessment, and treatment for clients at risk or living with HIV/AIDS. Certification in Reproductive Health (STIs) with the BCCNM, as applicable. A valid BC Driver's license and access to a personal vehicle for work-related travel.

Knowledge, Skills & Abilities

Comprehensive understanding of nursing theory and practice with a client-centered focus. Familiarity with community development and health promotion principles. In-depth knowledge of BCCNM Standards for Practice for registered nurses. Extensive knowledge of HIV/AIDS, STIs, and communicable disease management. Awareness of mental health issues and substance use treatment approaches. General understanding of Trauma-Informed Practice and Harm Reduction strategies. Proven ability to promote Indigenous Cultural Safety. Competence in providing culturally safe care across diverse populations. Strong knowledge of harm reduction strategies in the context of infectious diseases and substance use. Familiarity with community resources related to HIV/AIDS. Effective communication skills, both verbal and written. Ability to analyze situations, solve problems, and facilitate resolutions. Capacity to prioritize tasks, take initiative, and work both independently and as part of a team. Skills in managing challenging behaviors and crisis situations. Ability to identify community resources that align with client needs. Flexibility to adapt to new or unexpected circumstances. Proficiency in de-escalation and conflict resolution techniques. Leadership skills to provide consultation and direction. Competence in CPR techniques. Ability to operate relevant equipment and software applications. Physical capability to fulfill the duties of the role.
