Job Title: Clinical Coordinator, General - Palliative Care

About the Role:

We are seeking a highly skilled and experienced Clinical Coordinator to join our team at North York General Hospital. As a Clinical Coordinator, you will play a key role in providing palliative care mentorship, education, and coaching to healthcare teams within North York Region.

Key Responsibilities:

  • Establish relationships with community partners, organizations, and physicians to advance palliative care in the region.
  • Develop and implement quality improvement initiatives to improve access to and quality of clinical care, aligned with the Palliative Care Health Services Delivery Framework.
  • Provide palliative care coaching and mentoring to staff and clinicians in participating community organizations.
  • Collaborate with leaders of community organizations on local service integration and change management initiatives.
  • Consult on clinical care when the complexity of patients' needs exceeds existing competencies of providers.

Requirements:

  • Regulated healthcare professional with current registration in good standing.
  • Minimum 5 years of experience in palliative care.
  • Minimum 2 years of experience in adult learning techniques and knowledge to practice principles.
  • Experience working with quality improvement initiatives.
  • Strong clinical coaching and debriefing skills.
  • Strong consultation skills with expert knowledge of models of consultation.

As a Clinical Coordinator at North York General Hospital, you will have the opportunity to work with a dynamic team of healthcare providers, staff, and volunteers in a leading hospital. This is a part-time temporary position of 24 months with 8-hour shifts, days, and evening shifts in the Palliative Care Unit. We offer a highly competitive total compensation package, including pay in lieu of benefits, vacation pay, and the opportunity to join the Hospital Pension Plan.
