About the Role

Ontario Health is seeking a highly skilled and experienced Specialist in Palliative Care Implementation to join our team. As a key member of our organization, you will play a critical role in supporting the implementation of the Palliative Care Health Services Delivery Framework.

Key Responsibilities
  • Provide project management support for the implementation of the Palliative Care Health Services Delivery Framework, focusing on model of care recommendations for adults in community-based settings.
  • Coordinate and track regional work to ensure timely completion of deliverables, working closely with implementing organizations, such as Long-Term Care Homes, Ontario Health Teams, Family Health Teams, and Health Service Providers.
  • Support implementing organizations to advance palliative care through quality improvement initiatives, monitor risks, and sustain the spread and scale of successful initiatives.
  • Collaborate with regional and local clinical experts to organize palliative care education and coaching/mentoring programs.
  • Develop and maintain regional and local relationships to advance regional priorities and initiative goals.
  • Support the sharing of lessons learned and resources within the organization and across regions.
Requirements
  • Bachelor's degree in health science, health administration, business administration, or a related discipline.
  • Three years of project management experience, preferably in the healthcare sector.
  • Strong understanding of regional priorities and local healthcare needs within Ontario's healthcare landscape.
  • Knowledge of relevant legislation, including the Fixing Long-Term Care Homes Act and the Excellent Care for All Act.
  • Experience with palliative care initiatives and community health organizations or long-term care homes is an asset.
  • Advanced knowledge of Microsoft Office applications, including Outlook, Teams, Word, PowerPoint, Visio, and Excel.
  • Fluency in French is an asset.
