About the Role

We are seeking a highly skilled and experienced Specialist in Palliative Care Implementation to join our team at Ontario Health. As a key member of our implementation team, you will play a critical role in supporting the implementation of the Palliative Care Health Services Delivery Framework in our region.

Key Responsibilities
  • Provide project management support for the implementation of the Palliative Care Health Services Delivery Framework.
  • Coordinate and track regional work to ensure timely completion of deliverables.
  • Support implementing organizations to advance palliative care through quality improvement initiatives.
  • Monitor risks and sustain the spread and scale of successful initiatives.
  • Collaborate with regional and local clinical experts to organize palliative care education and coaching.
  • Develop and maintain regional and local relationships to advance regional priorities and initiative goals.
Requirements
  • Bachelor's degree in health science, health administration, business administration, or a related discipline.
  • Three years of project management experience, preferably in the healthcare sector.
  • Strong understanding of regional priorities and local healthcare needs.
  • Knowledge of relevant legislation, including the Fixing Long-Term Care Homes Act and the Excellent Care for All Act.
  • Experience with palliative care initiatives and community health organizations or long-term care homes is an asset.
