About the Opportunity
Reporting to the Health Services Manager, Chronic Disease Management and Wellness, Primary Health Care, you will provide expert knowledge and skills on a wide range of issues from advocacy to assessment to case management. As part of a primary health care (PHC) Diabetic education Centre team, you will deliver a variety of PHC services.
These services include, but are not limited to, health promotion, illness prevention, screening, treatment and monitoring of people living with diabetes. Your expertise may also be required to support individuals living with other chronic health problems, community development and advocacy. You will collaborate with others to increase community capacity as an educator, researcher and clinical practitioner.
Services are provided within a holistic, client/family centred care framework.
About You
- You possess a Bachelor of Science in Human Nutrition from a recognized university.
- You have completed a dietetic internship (a 1 year integrated or post graduate program).
- Current Membership in the Nova Scotia Dietetic Association is required.
- You maintain your Nova Scotia license through annual continuing education.
- Two years of clinical experience is preferred.
- One year of diabetes-related experience is required. If no applicants meet this requirement, those with less than one year may be considered.
- Membership in Dietitians of Canada is preferred.
- Current CPR/BLS certification is required.
- A Certified Diabetes Educator designation is preferred.
- You have experience with group education.
- You demonstrate strong time management and organizational skills.
- You are familiar with the principles of Motivational Interviewing.
- You exhibit creative problem-solving ability; you can work collaboratively with other disciplines and committees to achieve excellence in patient care.
- You are proficient in MS Office (Word, PowerPoint), email, and internet.
- You can effectively work both independently and within a multidisciplinary team environment.
- You possess excellent verbal and written communication skills.
- You demonstrate effective research and analytical thinking skills.
- You have a demonstrated commitment to diversity, equity, and inclusion.
- Competencies in other languages are an asset; French is preferred.
Compensation and Incentives
$43.42 Hourly
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
