Compassionate Primary Care Nurse

Are you passionate about providing exceptional nursing care? We are looking for a committed Registered Nurse who is dedicated to delivering empathetic patient care to become a vital part of our Primary Care Team.
In alignment with Northern Health's vision of an ideal healthcare system, the Primary Care Nurse (PCN) operates within a framework of Primary Care, ensuring that individuals and their families receive comprehensive services supported by collaborative interprofessional teams.
The PCN plays a crucial role within the interprofessional team, utilizing best practices to deliver competent, safe, and ethical care across various settings, including clinics, homes, schools, and community environments.
The Primary Care Nurse will be responsible for providing direct clinical care and services tailored to individuals, groups, and communities, ensuring that care aligns with the individual’s Primary Care Plan and personal health goals.
Work Schedule: Days - 08:30 to 16:30 rotating
Compensation:
The new wage is: $55.91/hour

Education, Training, and Experience:
- Current registration with the BC College of Nurses and Midwives as a practicing RN or RPN.
- A minimum of three years of recent, relevant clinical nursing experience in primary care and/or chronic disease management, including experience in patient education.
- Valid driver's license.
- Assessment and Intervention - Proven ability to conduct initial and ongoing client assessments and deliver nursing care through appropriate interventions.
- Communication - Effective communication skills with clients, families, the public, and medical staff, utilizing verbal, written, and digital means.
- Critical Thinking - Ability to analyze and evaluate data from multiple sources to solve problems effectively.
- Person and Family-Centered Care - Commitment to promoting care that is focused on individuals and their families, respecting diverse cultures and preferences.
- Educational Skills - Knowledge of adult learning principles to effectively educate patients and their families about health-related topics.
- Management Skills - Capable of managing time and resources efficiently to support service delivery and facilitate teamwork.
- Leadership Qualities - Encourages team morale and collaboration, fostering a positive work environment.
- Knowledge Integration - Utilizes factual information and research-based evidence to inform decisions and actions.
- Public Health Approach - Develops a foundational understanding of population health and harm reduction principles.
- Physical capability to perform the duties required of the position.
